We review all accident and incident forms every term but to be honest I can't remember last time we had to complete an incident form. Well you can bet that I have talked that up for today! Apparently there has been a case where a setting was thought to have removed pages to cover up an embarrassing incident. My current system (Pacey) provides a duplicate for parents which probably covers that issue so I'm still undecided and watching this thread with interest. This came up in an inspection long long ago - I was advised not to use the loose leaf system which we had a the time, but I could have a note book and number the pages and stick in our own devised forms into it.
