OOh Sunnyday, I haven't got a folder for incidents like fire theft or flood - could mean a trip to Staples!

Right - add to my 'to do' list.......

Can i just ask if you are creating your own forms do you use copier paper to create a second copy ?

No only one copy, signed by both the team member that has discussed the incident with the parent/carer and then signed by them too, this is then placed in the children's file.

I thought parents were supposed to sign and get copies of all these issues, for their records. My other problem is that if you put it in the child's file then how do you review what is going on and if there are any links?

We give copy of accident form to parents but nothing else unless they request it. We review all accident and incident forms every term but to be honest I can't remember last time we had to complete an incident form. Well you can bet that I have talked that up for today!

I've been thinking of a new system since our EY advisers mentioned recently that all pages should now be numbered. Apparently there has been a case where a setting was thought to have removed pages to cover up an embarrassing incident. My current system (Pacey) provides a duplicate for parents which probably covers that issue so I'm still undecided and watching this thread with interest.

This came up in an inspection long long ago - I was advised not to use the loose leaf system which we had a the time, but I could have a note book and number the pages and stick in our own devised forms into it. That is what we have done ever since.

Our incident sheets are loose leaf but we do number them. Accident record is the pre-school network book which has carbon copy sheets and we number.
