Rea Posted October 11, 2010 Share Posted October 11, 2010 Looks ok to me, if you dont mind i'll use it as a template for playgroup, not entirely sure what they've got on that front. You might want somewhere for an adult other than staff to sign that its a true representation of what happened/was said. You know if it was a visitor or a parent helper who had the initial concern.
