photo permission

does anyone have a photo permission slip for parents...have searched but can't find one....and too hot to be bothered writing a new one! :rolleyes:

we add ours to the registration form - parents circle yes or no - at least its one less form to fill out and file away.

for any parents that do not want their child photographed the key person makes a note as they are usually the person who would take the photo.x

Thanks guys! we already have one but it isn't detailed enough and we've had a few odd requests this year so thought i would re-jig. I keep all the permissions on one document but maybe i'l have a look at adding it on to the reg form

we used a separate one but have now included this onto our new admission sheet as so many forms !!!
