Childminder Cards

I have viewed a website recently which had some SOS cards on in case of an emergency. It said something along the lines of "I'm a childminder and the children with me may not be mine" it had space for a photo of the child and parental permission for medical care in case of an emergency. Does anyone know what I'm talking about, or where the website is?

I'm thinking the Bromley Childminders website would be the place to look.. Have had a quick glance but all changed since I looked at it last (years ago). I'll keep looking for now as their site's so much improved I want to stay there a bit.

I made my own, they have my name on them and the wording...."I am a Registered Childminder and the child/ren accompanying me may not be my own. In the event of an accident or emergency please contact.............."

 

They have my husband's details as the emergency contact. He has a copy of all my emergency contact details for the children I childmind, and their parents sign to give consent for this as part of my Registration Form. I, and they, felt more comfortable with this than me having their child's name, photo, DOB etc on the cards.

 

I also give the parents a card for their wallets/purses with my contact details on that says "I am a parent and my child/ren may be in the care of a Registered Childminder. In the event of an accident or emergency please contact........."

Not sure what the etiquette is on using other sites, but I'm going to load this so you can look up, Child Emergency Cards.

Honey its fine to link to other sites on here (unless the poster is using the Forum to publicise their own company/product ;) ), and giving the url is the best way so that if at any time the owner of the site wants to remove their documents, they can. The only problem comes when people upload actual documents where the copyright is owned by someone else which can potentially cause the Forum a problem. I had a quick look at the Bromley site, and I'm sure giving a link to their available downloads doesn't constitute 'distributing' the documents.

 

If there's a problem, our Key Dude will no doubt let us know! :P

I couldn't attach mine from Publisher but here's what I could cut and paste. They're just missing the logo I use, they're the size of a business card and I laminate them to extend their life!
