mrsbat Posted April 6, 2016 Posted April 6, 2016 Yes we do, it's on a format devised by our LEA (Norfolk) and it includes children who have ....... play plans/ISP's/Behaviour plans, those with individual risk assessments, children with FSP's, those with safeguarding concerns, EYPP, 2 yr funded children, LAC, Those under SLT and those with safeguarding/child protection concerns. Mouseketeer Posted April 6, 2016 Posted April 6, 2016 Hi, yes we do (well a sheet, rather than a register as such) off the top of my head it just has who, area of concern (if not already known on admission), date discussed with parents and agreed action ie contact HV or S&L referral etc.., any other professionals involved, dates of appointments (so we can chase when we haven't received our copies of reports, which is quite often), not sure I have one here, but can send you one tomorrow when i go in to work if it helps.
