Stargrower Posted June 12, 2015 Posted June 12, 2015 I don't use a form to review mine. I look to see if the forms show a pattern in accidents etc then use our risk assessments to record any changes we make as a result of reviewing the forms. 3 Quote

Foreveryoung Posted June 14, 2015 Posted June 14, 2015 Mines not a form its piece of plain paper lol. I do a tally chart of the type of accident and area of accidents over a half term period. Then I write a statement at bottom I.e physical area increased accidents occurred Etc etc etc' then if any action required, this statement is then shared at staff mtg. x Quote

We had a complaint regarding an accident a while back and this is something that they asked for evidence of and wanted to know how often we undertook them...
