Guest terrydoo73 Posted September 8, 2012 Share Posted September 8, 2012 I know this is a huge ask but would anyone be willing to share with me a parents questionnaire they would give out at the beginning of each year or when a new child starts at the setting so that you can follow a child's interests more clearly. We had an Early Years Specialist with us yesterday who asked the question "how do you know what your children's interests are? do you send out a form to the parents or what?" We of course looked stumped. We do have an All About Me form but it doesn't go into specifics rather focuses on their family details, what food allergies they may have, if immunisations are up to date, whether fully toilet trained etc but there is nothing on there about liking animals specifically whatever, enjoys Dora, would prefer to play on own or enjoys water/painting/sand etc etc. Just something simple that I could hand out to parents and get a quick turn around would be brilliant otherwise I will have to sit down and write something which could be not relevant in any way shape or form! Have decided to go with the simple child questionnaire as it is succint and just what we need at the moment - not that I don't think the others are good but I think for our setting the child questionnaire is more appropriate. We have sent out so much paperwork over the past week with little response that I think I might keep this to our Parents Meeting and ask them to complete before leave - of course I will have a duplicate of all the other non returned forms for individual parents to complete - makes my job doubly hard as I am doubling being a Leader and an administrator!!
