Transition To Another Pre-School

One of our children has moved house and so she is moving to a private nursery closer to home.

 

I have invited her new key person into our setting and she is visiting in July.

 

Does anyone have a copy their transition form which you send to the new settings?

We would use the same as our local authority one which we send to primary schools - do you not have to do that?

It's the last few pages in here. I hope it's of some use, I will be re-doing the seven areas section but this should suffice for now.

we send their learning stories (with permission) and a feedback form(report) with a summary of what they have done that term and what their next steps will be. (we do feedbacks one a term and they are all kept in the learning stories.Our lea dont do transition forms.
