I've bought 'Sarah's' ebook which is great but I thought I saw that someone on the forum had kindly offered to share a completed form with members to provide an idea of what is required, but I can't find it now! If you click on Search Forum Posts and then enter 'Exemplar Sefs' you should find what you are looking for! Exempar SEFs is the one she is talking about SEF for childminders may also be useful and if you do try the search there are many others too.
