I'v had a look on Amazon for books which might help to introduce the subject, particularly the idea of going to sleep while they do it but I couldn't see one which clearly fitted the bill. They do a 'first experiences' range of books and I'm pretty sure one of them must be about going to hospital. Not sure how appropriate they'll be for a big grown up five year old though!
