Ticklist for parents of children starting at preschool

Hi, does anyone out there have a proforma of a ticklist that parents can complete prior to their child starting preschool? We are looking at amending ours as parents seem to want to tick a box regardless of whether their child can do something. Perhaps giving them 3 options for different stages of one area so they can tick a box for their child and give us a more realistic idea of where their child is at. I just wondered if there is a form like this in use out there already.

Hi we have an area on our personal details forms regarding toileting, sits, stands, potty, toilet, nappies, pull-ups etc.

We also have a "all about me" sheet which parents complete prior to their child starting .

This has things like family, pets, likes, dislikes, this goes in their Learning Journal.

All other information is gathered once the child has started and more importantly settled with us.:)

We give parents an information form to fill in when they come for their trial sessions with simple 'yes' or 'no' questions and we do a baseline tracker for our key children during the first half term. Sounds like you're after something a bit more specific but here's a copy of our form anyway, in case it's useful to you. :1b

we do a welcome and getting to know you form followed by a home visit so the parents can tell us about their child's home life. We seem to have the majority of parents valuing this and the children love welcoming us to their home showing us there toys and favourite places.
