Guest Posted June 25, 2010 Share Posted June 25, 2010 Does anyone have a form where they log child protection concerns?? Either marks/bruises we have noticed or things the children say (disclosure). I've seen in the past a form with pictures of a body where you can mark bruises etc. I would be very grateful if someone could put one on here for me to look at. Thank you in advance! I already have an existing injuries form but needed a form for injuries we may notice or disclosures from children. I hope others find it useful.
