Hi

Can anyone help me please?

Did I see an A4 sheet which parents filled in with their child or about their child which could be then used for planning.

eg favourite food,toys...what makes them happy? sad?

I have got yours and I do like it .I would like something on just one sheet as I don't think my parents will go for the long version!!!!

Heres mine...its given out to two children a week (on a rota basis) the key worker fills in the top bit and the parents the bottom. The information is then used to inform the following weeks planning!Could you adapt the titles of the boxes???

Have to say that I use Caits....parents have always been very good about completing it.......

The shorter version looks great for ongoing observation but I wondered if I could borrow yours Cait for my parents when the first join?

Here is the one I use which works really well for us. We put it at the beginning of their Learning Journey Books.

I really like that - thank you so much for sharing!

 

A quick edit to say........I'm going to use this for my 'returning' children...........obviously things will have moved on/changed since they completed their 'My Unique Child' entry forms....

 

So thanks again - I think they're great!
