Parent Carer Observation Format

We have a space on the bottom of our observation forms for parent input, the keyperson then talks to the parent about the observation and next steps and gains input from the parent that way.

Hi

 

We have a basic sheet that states the benefits of parental involvement in observations and that gives a big space for them to write whatever they want to. We let them complete one however often they wish *(which isnt often)

 

To try and get them even more proactive we have a wall display that has lots of luminous star cards pinned to it, we call these 'WOW' cards and get the parents to quickly write something their child does or has said etc and at the end of the week, we take the complated wow cards and put them in the childs learning story. This gets more of a response than the sheets.

Hi. We don't have observation sheets for parent input, but like the idea of 'wow' slips and have been trying to design one

for the past week. The luminous stars sound a quick and effective way of gaining parent input but is there anyone who would

be willing to share their 'wow' form with me? :o

We have two forms-one is special news for if they have done something over the weekend or during the holidays and the other is a wow form, for those moments such as when they have learnt to peddle a bike or they have slept in their own bed all night (more of a wow for parents I think!!!) I will attach them for you. They are very simple and parents can add a photo if they want.
