Hi Skippy,

 

Are you thinking staff conversations?

 

If so, i have just put this in place and it has been a godsend. I literally have devised a table that includes a box for each:

 

Name of staff member

date

details of discussions

Follow up

 

I get two on an a4 page and then add them to staff supervision folders.

 

If I am on the wrong track, ignore me although you could use a similar format for parental conversations.

We record general parent conversations on a piece of paper, page per child, which is kept in a folder & all staff can see (providing it's not confidential) - this folder is then emptied into the child's records once the child leaves.
