Ours is the one issued by our local authority - so the codes used tie into the information they have to provide.

We dont ask new parents, should we be?? However when fillng NEG forms we are required to and it is issued by our local authority

We use a 'coded' one form our LA - we just send it out with information when a child starts - it up to the parent/carer as to whether or not they return them.

We only use the information for the NEG funded children.

there is guidance in eyfs section on documentation on recording ethinicity. We have a section on our registration forms for parents to complete if they wish. However we were told we could not ask them to fill it in it was their own choice to do so
