Self Evaluation Forms

I am thinking of producing my own self evaluation form, rather than using Ofsted's version. Does anyone else have their own or any ideas/

Ok, found the link to peggys.... because of the title it doesnt appear when you search for self evaluation...she called it assessment!

we use these to justify our decisions but Ofsted did want to see thier form filled in when they came, if it is not done I know one group who had to fill it in while they wait!!
