Recording Bruises/injuries That Happen At Home

Please can someone tell me what format/forms they use for recording injuries that children come to the setting with.

 

I realise I need to update my system and want to ensure I go about it in the correct way

 

Do you use your own forms/books. If so can you point me in the direction of some

 

I am looking for some good advice

It is always worth doing a forum search Glen. There was this thread very recently which discussed this issue, and some members uploaded their forms.
