toddlermaddness Posted March 27, 2012 Share Posted March 27, 2012 Hello all, I am looking for a form that can be distributed to staff before an appraisal. Not an appraisal form but a form that is asking other staff to comment about the appraisees performance. Its called a 360 degree feedback process.
