Any Have A Feedback From For Appraisals Please?

Hello all,

 

I am looking for a form that can be distributed to staff before an appraisal. Not an appraisal form but a form that is asking other staff to comment about the appraisees performance. Its called a 360 degree feedback process.

 

Would like to see if anyone else uses it and if anyone has a template can help me start one up.

try on business balls ....or do a two stars and a wish form which we have used is simple and effective, gives staff the chance to be honest and to criticise in a constuctive manner (ofsted really likes them)

thanks for your reply! Two stars and a wish form? Is this two stars for what you see them to be good at and the wish is something you think they should be or could do, what you wish for? lol

yep absolutely... i think there's one in the resource library or just google it will come up!
