fimbo Posted November 10, 2011 Share Posted November 10, 2011 when i was promoted to playleader a few months ago -should I of completed an EY2 form for Ofsted ? as my chairperson has told me that she's filled it in and sent it off ?? im a bit confused -as i still havent had a suitability check -or have even been contacted by Oftsed? Inge Posted November 10, 2011 Share Posted November 10, 2011 (edited) yes.. you should have completed a form which will trigger the other stuff Ofsted do , cannot see how she can complete a form for you as if I remember correctly it asks for your signature on it and info on you she may not have access to.. or need access to.. actually just had a look at the form and it asks for references, previous names and addresses ,medical declaration and lots only you can answer.. Guest Posted November 13, 2011 Share Posted November 13, 2011 I would suggest it is the Ey3 form as that never seems to trigger any action form Ofsted. The EY2 can only be completed by you and it triggers the CRB form to be sent from Ofsted. They tend to contact you regularly if you don't complete it and return that.
