Guest Posted August 22, 2011 Posted August 22, 2011 Hello - does anyone have a form/questionnaire that they ask parents to complete if their child has asthma and that they would be willing to share? It would need to include things like: How severe, what are the triggers, name of medication, will medication be kept at school? etc. I used such a form for parents many years ago when working in a pre-school, but have nothing in my school nursery; I didn't keep it when I left and my memory is not what it was!
