Sue R Posted June 16, 2007 Posted June 16, 2007 Not got one to hand, but I would think all you need is child's name, nature of accident with circumstances, treatment already received and details of any special considerations for the setting, signed and dated by parent/carer and staff receiving child? Lou Posted June 16, 2007 Posted June 16, 2007 We use our accient book to record the injuries and the parent or carer signs and takes a copy. If the child makes a coment about the injury during the day it gets recorded in our incident book. Quote

Guest Posted June 17, 2007 Posted June 17, 2007 I also record in accident book, name, age, time of report, details of injury, any medication given or any medical aid, ie doctors / hospital. Only comment from Ofsted about my records was staff signature, I was only using staffs first name, we should use full name including surname because records are kept for such a long time, staff turnover etc would we know who 'Sarah' was 10 years later, should we need to.
