Guest Posted May 14, 2007 Posted May 14, 2007 Does anyone have a consent form which they would like to share giving permission to apply nappy and ezcema creams and lotions? Would be interested to see examples. Thanks. Sam
Guest Posted May 14, 2007 Posted May 14, 2007 We don't have one for nappy cream and excema cream is only administered if prescribed, and then we follow our medicine policy. Peggy Medicines_CONSENT_FORM.doc
Guest Posted May 14, 2007 Posted May 14, 2007 Surely this should be the same as any medication you apply/administer? We have a consent form for any "medication" we use on the children which includes inhalers, creams or anti-biotics etc. You would have forms coming out or your ears if you had to have a separate one for each ailment. If you have an administration of medicines form then I would suggest you use that. Linda
HappyMaz Posted May 14, 2007 Posted May 14, 2007 You would have forms coming out or your ears if you had to have a separate one for each ailment.Linda But you've probably got a policy to cover administering medication for that particular eventuality, haven't you...? Maz
Guest Posted May 15, 2007 Posted May 15, 2007 Thanks for responses. With regards to nappy rash cream which isn't prescribed, i wanted parents permission to apply in the same way as we request permission to apply sun protection lotion which all parents are asked to give permission for. So sadly - made another form But yes, you're right ezcema lotion should be prescribed so can go on existing medication form and is therefore covered by our policy. Thanks.
