Reviewing how we record observations

Hi,

I am in the process of reviewing how we record observations and was wondering does anyone have any samples of how they record these. I am based in N. Ireland and we still use paper sheets for recording observations. PLEASE HELP!!!!

We have just moved onto tapesty I think it is going to be brilliant although staff still need to write meaningful observations.
