Advice needed re: MTA Staff Supervision

I need advice with regard to carrying out staff supervision for mealtime assistants within a school nursery. I am in the process of updating the forms which are currently used and am starting with a 'blank sheet' - but so to is my head!! :blink: Does anyone have a form or any thoughts they would be willing to share? I need the form for ... ... tomorrow! :o

Any thoughts would be SO appreciated :wub:
