Panders Posted March 2, 2011 Share Posted March 2, 2011 I am in the process of devising one at the moment, we have looked at a few examples and the staff have had some input into the type of layout they feel happy with, just need to collate all the info and put one together! Guest Posted March 3, 2011 Share Posted March 3, 2011 Hi, we have used the following for the past 2 years and found it works well. It is cobbled together from various ones on here. Staff fill out the first part ahead of the appraisal, you then work through it together (you need to take a photocopy) in the appraisal.
