Just wondered how often you do them.

Twice or once per year.

Do you use the same format?

Anyone got any they would be willing to share?

Hi, twice a year (in theory!), here's one I've cobbled together from others on here.

I took the chance to add in a question about why people chose our preschool, that was to see how well our advertising and marketing worked, or whether it was just about word of mouth.

 

I choose a moment when parents are likely to be getting minimal paperwork, e.g. January, so they are more likely to complete it.

We do children's info 3 times per year ( just what do you like etc) to put on their reports/feedback and we used to do a leavers questionnaire but now do 3 stars and a wish forms 2/3 times per year
