Foundation Stage Long Term Plan

Hi, I was going to create a chart which shows the Development Matters and Early Learning Goals, Learning Intentions and planning and resources for the 6 areas of learning to make it easier when planning as I thought if these 3 areas are clearly set out on one page as seperate headings it would help with my planning to ensure I am covering/meeting the statutory requirements. I just wanted to ask before I do this if anyone has already created a template similar to this. I have checked on the site but cannot find anything.
