End Of Year Self Assessment Sheet For Children

Does anyone include a self assessment sheet which the children fill out (or adult scribes) about how they feel the year has gone to go in with the report?

 

I have just remembered that last year I was asked to do this, and totally forgot for this year and can't find what we used last year. Just wondered if anyone has a useful form, things like 'Things I have enjoyed about Reception...'

My class have a bit to do for their reports - just an A4 page with a few lines at the top (depending on their writing size, etc) and they write something they like doing, something they have particularly enjoyed, something they have learnt, etc. Most children have written a couple of sentences, or more for the most able, but my least able few have just done one sentence (with help).

We have an A4 page folded to create four A5 sides. The first side they write their name and draw a self-portrait, next page they draw/write their friends in Reception, next draw/write what they like doing and school and final side draw/write what they are looking forward to in Year 1.
