SueJ
This is the one that I use. It is tagged at the end of our annual report which goes into quite a bit of detail about what we have been up to during the year to remind everyone. When the questionnaire responses are back I follow up with an analysis of the answers and what we intend to do if there are any major things e.g if more outings are wanted how we will try to address that need. It is a mix of yes/no and comment and we get quite a good response back. The children's questionnaire was used for the first time this year based on the Ofsted questions and the responses were lovely. Guest
I haven't got time to find it this morning but we did a parent and a child questionnaire, and made the child one very easy with a smiley face or unhappy face idea for rating the activities, the cafe, the staff etc!!
