This is an enrolment form that, in our view, meets GDPR requirements


This enrolment form was written ‘in-house’ by the FSF team. It has not been checked by lawyers and as such represents the FSF’s best interpretation of the GDPR as it applies to early years settings. Settings should decide whether they agree with the interpretations made before using the enrolment form. The enrolment form can be adapted to suit the specific needs of the setting but the FSF does not take any responsibility for the accuracy of any of the information included in the original form. Once changed, the form no longer represents the work of the FSF team.

V2. Added sentence after additional adults details:

"Please ensure that you tell any additional adults that you have given us their contact details so that we can contact them if we are unable to speak to you if your child is unwell, has an accident or is uncollected at the end of the session. If they have any questions or queries regarding this, please direct them to contact xxxxxx on xxxxxxx. Thank you"

V3. Added a watermark

V4. Added "We will not use these details until your emergency contact has filled in the ‘emergency contact consent form’ and it has been returned to us."
