For the purpose of uploading I've removed all names but in our document it uses names and telephone numbers listing the CMT and then initials throughout the document. We also have been issued with credit card size laminated cards with phone numbers etc. It has proved useful on occasions! like this... this is a school one but I'm sure it could be adapted.
