Guest Posted April 21, 2007 Share Posted April 21, 2007 Well I'm guessing that the usual standard incident form may be too small, as you may have a bit to write up. Include: Date, time, child's full name and DOB, what happened, how it was dealt with, signature and name of staff members, signature and name of manager, signature and name of parent informed. Guest Wolfie Posted April 21, 2007 Share Posted April 21, 2007 I agree with everything Shelley said - and yes, it would be good to document how you have learnt from the situation and what steps you will now take to make sure that it doesn't happen again. When I ran an Out of School Club, I found that I had to implement all sorts of policies and procedures to make sure that all children were where they should be and that everyone KNEW where they should be at all times! Effective communication between parents, the school, the Out of School Club and the children themselves is vital and I found that most problems occurred when it broke down for one reason or another. Some of my procedures evolved as a result of crisis management of situations like the one you describe so I know exactly where you're coming from!
