Incident Format

hi we have had an incident on a school site out of school club where a child left the school grounds and was brought back in by a parent.

I have to write up an incident report and wondered if anyone had a format I could use. Thanks in advance.

Well I'm guessing that the usual standard incident form may be too small, as you may have a bit to write up.

 

Include:

 

Date, time, child's full name and DOB, what happened, how it was dealt with, signature and name of staff members, signature and name of manager, signature and name of parent informed.

 

Would also be good to add on any measures that need to be put into place to prevent it happening again.

I agree with everything Shelley said - and yes, it would be good to document how you have learnt from the situation and what steps you will now take to make sure that it doesn't happen again.

 

When I ran an Out of School Club, I found that I had to implement all sorts of policies and procedures to make sure that all children were where they should be and that everyone KNEW where they should be at all times! Effective communication between parents, the school, the Out of School Club and the children themselves is vital and I found that most problems occurred when it broke down for one reason or another. Some of my procedures evolved as a result of crisis management of situations like the one you describe so I know exactly where you're coming from! Who are you writing the report for - yourselves or someone else?

I would agree with the above, maybe use the same format, and use the back too, or type it up as a report and attach to the original incident report.
