Good Examples Of Cross Curricular Planning

Has anyone got a really good method / planning grid which will help me to plan for foundation subjects as a cross-curricular topic block that they could share with me please?

 

I've experimented with a few ideas, and have a medium term overview. The trouble is, the short term planning - I just don't seem to have it right yet.

 

Any help will be gratefully received.

I dont have any of my own but the hamiltontrust.org website has crosscurricular planning for Early years and KS1 and 2. Some teachers who use them really like them.
