I'm in Reception and this year I hope to have parental interviews e.g. so that they can contribute towards FSP. Does anybpdy have a format or any questionares they have previously devised, used etc. I would be very grateful if u could pass them on. Thanks in advance. xxx Flower

Susan Posted August 14, 2006 Posted August 14, 2006 Yes! The FS profile handbook has suggested guidelines for an initial questionnaire/consultation. I made the following with reference to it although I stressed to my team that it was to be used to guide conversation when we went to the home and it may not always be appropriate to ask all questions, they should be sensitive to what they saw and to the parents etc.. It certainly helped to make sure the same sort of information was gathered by all of us! home_visit_questionnaire.doc
