Tracking Ss And Elgs

Hi, I know I have seen this somewhere - has anyone got a document which has the SS and ELGs going across the top, and a class list down the side, which is used for recording when children achieve the SS and ELG's. I don't mean the one with the profile points, but the one which covers all the SS. If anyone has a copy I would be SO grateful.
