Individual child risk assessment

hello does anyone have a template for a child requiring a risk assessment

We have a child who runs and hides and also displays very physical behaviour

No I haven't sorry , will not your policies for the setting be enough? Perhaps write a care plan instead. Or even an IEP if strange behaviour is being displayed for evidence if needed at later date for outside agencies .

Yes we have policies, IEP, but have been asked to provide an individual child risk assessment

I've got one for a child who is autistic and did quite a lot of hitting adults and children. Is it just the template you want? I made it up based on my usual risk assessments for other things. Let me know if you think it might help.
