Leavers Questionnaire

Hi All,

 

I have been thinking for a while now and just wondering if anyone else does questionniares for the children's parents/carers who are leaving or left.

 

Can any one help me please?

In schools we have regular annual parents and childrens questionnaires rather than on leaving.

We're a preschool and we do a leaver's questionnaire but find the only ones who fill it in are the ones who leave in July and even then we don't get many back.
