Child Survey

Does anyone have a pro forma for a survey that I could send home for the children to say what they most like doing?

I do have some ideas that it would need to have lots of pictures and happy/sad faces.

I seem to recall that ofsted have a sheet on questions to ask children we used it as a base for ours but I have to say the parents answered the questions though

I've got that sheet somewhere - will try to find it. I used it in setting with Pre-school children (day nursery). Got some fantastic replies which I used as a dsiplay with photos of our children engaged in activities etc - looked great and got parents talking. To each other and us!

I like the question about 'does anyone else live here?' since I'm beginning to think the children are right and I actually do live in school!
