Sorry I haven't got one to share but didn't want you to think your request was being ignored

 

We haven't taken on a new member of staff for years and sorry can't remember using a form when taking up references.

Hopefully someone with helpful suggestions will be along soon

There is one on here. It is quite detailed but you could amend it to suit your own circumstances.

Thank you Geralidine and Beau for your replies. The website link was very useful, I will adapt it as suggested. I had a vague idea that one has to be careful what one says in a reference and this seems to give a clear explanation.
