Risk Assessment For Climbing Frame

Hi,

 

We have had a slide and climbing frame for about a year now and had untill recently been adhering to a generic risk assessment for playgrounds. The other day one of my children fell off the slide. Fortunately he was unhurt thanks to a spectacular roll and the special floor surface.

 

It happened after he was pushed while his legs were hanging over the side- we had talked about how we go down the slide safely but children will!!!

 

This is not mentioned in the risk assessment and I was wondering if anyone had a better one that I could look at which was aimed for FS equipment?
