Transition Toolkit

I am putting together a Transition Policy and in my research found reference to a Transition Toolkit. Can anyone tell me if there is a special document? I have looked but can't find what I need. Thanks

Hi Lesley

If you know any setting in the Poole area we had a Toolkit if I find the CD I'll email you the transition stuff you can always PM me and I'll try and explain it if i can't find it. If not if you come into Poole at any time I'm in the town centre area and you could copy it
