Guest Posted November 17, 2008 Posted November 17, 2008 Hi all Have got to do a referral form for child health assessment (patch) and am a little unsure what I am asking for - what is this a request for - is it for the child to be seen by a paediatrician? Being really thick I know!!!!

Being really thick I know!!!! My Inco has suggested it and given to me to complete we had a meeting with the parent and the parent requested help. It is for behaviourial concerns - I just wasn't quite clear what this is asking for

It is for behaviourial concerns - I just wasn't quite clear what this is asking for just to clarify my question - who is going to assess the child and will this be at home at the nursery or at the hospital?
Guest Posted November 17, 2008 Posted November 17, 2008 I haven't heard the term "patch team" before but in our area we have a clinic at the hospital which children can be referred to for a multi disciplinary assessment. They include Ed Psych, SALT, hearing specialists, etc. The child attends at the clinic and if any needs can be, they are then dealt with in the community. Otherwise they continue to attend at the clinic at the hospital.
