Has anyone got /done one and would you share it please? Helen Posted September 26, 2008 Share Posted September 26, 2008 These are my Personal Development Plan self-evaluation form, and appraisal report form. When I ran my own nursery, we used this once a year. The staff member filled in their one, I filled in my one, and then we met to discuss them both. Finally, I drew up a summary of what was agreed, etc. and we both had copies.
