Reception Observations

Hi,

Is anyone out there willing to share their observation proforma for a Reception class, which ticks all the boxes in the EYFS? Last year I used the famous post-it method, but when I was moderated I was told i need actual observation sheets as well. How are we expected to link this to the principles/ commitments? Getting a tad confused!!
