We have been using blank pages but I am keen to have a format and produce a ringbound book so that all our logs follow a similar style....... Thanks in advance! I'm also going to include a form I found on the site here 'Record of Existing Injuries' as a separate section in the folder and also a section on 'Notification of Infectious Diseases' as all this would come under the Safeguarding umbrella. Running Bunny Posted October 3, 2007 Share Posted October 3, 2007 The sample that I have includes very similar details, although it is aimed specifically at behaviour. One thing I would have is the name and signature of the first person who dealt with the incident and then the counter signature of the manager/most senior person on site at the time. This will demonstrate that management had been informed,whilst the person dealing with it has the first hand account. Apart from that, it all looks fine!
