Guest Posted June 28, 2007 Posted June 28, 2007 Hope you can help me. We are reviewing our staff self-appraisal form, at the moment ours is basic with 'What are your strengths/weaknesses' type questions . We hope that our new form will indicate more clearly areas where staff feel that they need more training etc. I have thought about creating a grading system covering a range of topics and areas, but am not sure how much detail to go into. So does anyone have any they forms that they are willing to share, even if not on a grading bais. All help would be welcome Karen Quote
Guest Posted June 28, 2007 Posted June 28, 2007 At my old setting our self appraisal forms contained a list of our responsibilities / duties and we had to fill in if we thought we had fulfilled them or not and why. I have attached the supervisor's one. 15_Form_for_self_appraisal_by_staff_membersupervisor.doc Quote
Guest Posted June 30, 2007 Posted June 30, 2007 Thanks Suzanne , it looks really useful and I will adapt it to suit our setting. Does anyone have one that is more of an overall form for all staff, including how much they understand aspects such as child protection etc? Thank you Karen Quote
