It's from the Bobby Shaftoe book by Sue Nicholls.

Witch's cauldron
Here's the Witch's hat,
Witch's broomstick,
Witch's cat,
Fly over here,
Fly over there,
Crash, bang, Alazam!
Now you disappear.

Witch's spider,
Crawling up a log,
Witch's spell book,
Witch's frog.
Hope over here,
Hop over there,
Crash, bang, Alazam!
Now you disappear.

There are actions too.
