We have one simply because we have a lot of SEN children and as Manager I like to have somewhere to look when I need a quick overview. Ours is a table similar to the one Sunnyday has posted. Sorry i cant add an example, but from memory the columns are:

Name

Parent and family details

Area of concern

Professionals involved (including contact details)

Days and times of attendance

Name of any other setting attended

Next review date (for 1 Page Profile, My plan, TOPS)

Notes (these are very brief though, just key points)

COP is short for Special Educational Needs and Disability Code of Practice. It was not a silly question at all, I think it is silly that we constantly abbreviate everything - but them I have just realised I have used 2 in my post above!
